New Patients
Before you are seen by the doctor, you will be need to fill out a form which asks for personal information, your insurance policy number
and your past health history. Please accept our assurance this information is kept confidential.
Fees/Insurance
Patients are expected to pay for office charges at the time services are rendered. This may involve payment for office services or
payment of a co-pay or deductible by your insurance agreement. As a courtesy to our patients we will file your insurance claim.
All policies are different however and you are responsible for any amount which is not covered by your insurance company. Should
you have any discrepancies with a claim, please direct them to your insurance company before contacting our business office. This
will normally speed up correction of the claim. We accept Medicare assignment and will file with your supplemental insurance company
when appropriate. If you change insurance carriers please notify our receptionist to assure prompt resolution of your claim.
